Thank you for choosing Klinik Dr. Sophia Y and allowing us to be part of your healthcare journey. To refer a friend to Klinik Dr. Sophia Y, please provide their details under the “New Patient” section, along with your details in the “Referral Patient” section. As a token of our appreciation, “Referral Patients” will receive a one-time cash reward for each new patient appointment (T&Cs apply).
What is this programme about and what will I receive?
This Referral Programme is for patients who have previously visited Klinik Dr. Sophia Y. These patients (hereby known as “Referral Patients”) will be rewarded when they refer new patients to the clinic (upon completion of their first appointment).
1. Eligibility
1.1 The referral program is open to all existing patients of Klinik Dr. Sophia Y. (“the Clinic”).
1.2 Referrers must be patients who have completed at least one treatment at the Clinic.
1.3 The referred individual (“Referee”) must be a new patient who has never received any treatment at the Clinic.
2. Referral Process
2.1 Referrers must provide their Referee’s contact details to the Clinic using the official referral form available at the Clinic or online.
2.2 The Referee must complete a qualifying treatment to validate the referral.
2.3 Qualifying treatments include any aesthetic procedures offered by the Clinic.
3. Rewards
3.1 Referrers will receive a reward for each successful referral. The specific reward details will be outlined in the promotional materials and may change from time to time.
3.2 Rewards are non-transferable and cannot be exchanged for cash or any other form of compensation. 3.3 The reward will be provided within 30 days after the Referee’s qualifying treatment is completed.
4. Limitations
4.1 Each Referrer can refer a maximum of 5 new patients per calendar year.
4.2 Referrals made to immediate family members (spouse, parents, children, siblings) are not eligible for rewards.
4.3 The Clinic reserves the right to disqualify referrals that are suspected of being fraudulent or non-genuine.
5. Privacy and Confidentiality
5.1 The Clinic will handle all personal information in accordance with its privacy policy.
5.2 Referrers must obtain the consent of the Referee before providing their contact details to the Clinic.
6. General Conditions
6.1 The Clinic reserves the right to modify or terminate the referral program at any time without prior notice.
6.2 Participation in the referral program constitutes acceptance of these terms and conditions.
6.3 The Clinic’s decision on any matter concerning the referral program is final and binding.
7. Liability
7.1 The Clinic will not be liable for any loss or damage arising from participation in the referral program, except for any liability which cannot be excluded by law.
